ACGME ACCREDITED HOSPICE AND PALLIATIVE MEDICINE FELLOWSHIP - 12/22/2008 9:02:33 AM
The need for trained physicians in hospice and palliative medicine continues to rise as the population of the United States faces the challenge of providing care for a rapidly growing population of patients and their families facing chronic serious and life-limiting illnesses.
- In 2005, 12% of the US population, 37 million adults, were age 65 or older
- By 2030, 20% of the US population, 70 million adults, will be age 65 or older
- At the present time, there are more than 4300 accredited hospices in the United States
- Between 2001-2006 there was a 60% increase in the number of nursing home residents enrolled in hospice
- In 2006, 1.3 million patients, 36% of all patients who died in the U.S. were under the care of a hospice program
- Three-quarters of those living beyond 65 years will contend with stroke, cancer, heart disease, COPD or dementia within their last year of life (JAGS 2000; 48(5suppl): 5214-5221)
MetroHealth Medical Center's one year ACGME Accredited Hospice and Palliative Care Fellowship Program is based in the Division of Geriatrics and Palliative Care within the Department of Medicine. The program exists in conjunction with the Residency Training Program in Internal Medicine. This one year program is a collaborative effort between The MetroHealth-CASE faculty and Hospice of the Western Reserve. The fellowship is open to applicants trained in internal medicine, family practice, anesthesia, obstetrics and gynecology, or emergency medicine. Two fellowship positions are available per year. ACGME accreditation is pending.
MetroHealth Medical Center is the key health care provider to the uninsured and underserved population of Cleveland which is home to over 2 million people and 60 ethnic groups. MetroHealth System includes a major academic medical center, rehabilitation hospital, two long-term care/skilled nursing centers, outpatient surgery center and a network of community-based healthcare centers. MetroHealth is a leader in trauma, emergency and critical care, comprehensive medical and surgical subspecialities, heart and vascular care, cancer care, senior health, stroke and rehabilitative services.
Hospice of the Western Reserve (HWR), a communmity based Medicare certified hospice program is the fourth largest in the country and has a daily census of 1200 patients who receive services from 20 transdisciplinary teams in ten sites. A specialized pediatric team has an average daily census of 40. Care is provided wherever the patient calls home-individual dwellings, long-term care facilities, assisted living, group homes, or Hospice House, HWR's residential faciilty with general in-patient beds used for symptom management. The patient population at HWR represents a wide variety of disease conditions with greater than 50% having a non-cancer diagnosis.
The MetroHealth-CASE and Hospice of the Western Reserve fellowship program provides experience in:
- Outpatient palliative care clinic
- Inpatient hospice care
- Acute hospital palliative care consultation
- Training in the interdisciplinary team model
- Long-term care/geriatrics
- Physician house calls
- Community-based hospice care
- Acute symptom management
- Pediatric hospice and palliative medicine consultation
Additional rotations include:
- Geriatric medicine
- Interventional pain management
- Physical medicine and rehabilitation
- Pulmonary critical care
- Oncology
- Radiation oncology
- Pediatric comprehensive care
- Pastoral care
- Integrative therapies
Formal education on a wide variety of topices is provided through:
- Weekly didactic sessions
- Seminars
- Grand rounds
- Journal club
